Registration form Print and mail or fax

Dear Earth Central Participant:

You can now register for the credits for the STEM Earth Central course
through Continuing Education. I have attached a registration form that you
can print and return by fax or by postal mail. You may also register by
telephone. 

The deadline for registration is normally earlier, but because of the delay
in getting the paperwork set up, it had been extended to September 30.

You must fill in the "Class No." and "Subject and Course No." Your choices
are

40427     GEO 590F
or
40428      EDUC 615A

I have filled in the course title, credits, and fees.  Omit the UMass ID
unless you have one. The postal address plus the fax and voice telephone
numbers are on the form. 

Copies of this information and the registration form are also available on
our course website, www.umassk12.net/earth. I can also send you one by
postal mail if you cannot access the .jpg file.

Dear Earth Central Participant:

You can now register for the credits for the optional extra 3 credits for
the STEM Earth Central program for the dissemination or  research options.

To do this, just add this line to the registration form you received
earlier:

  Class # 40518, GEO-SCI 597F, Earth Science Research, 3 credits, $225.
  
  Cross out the old total and enter the new total, which is $225 + $225 + $35 = $485.

I have attached a registration form that you can print and return by fax or
by postal mail. You may also register by telephone, 413-545-3653 or
800-922-8211, 8:30-5:00. You can also fax the form which you received
earlier; a copy is on the STEM Earth Central website. The postal address
plus the fax and voice telephone numbers are on the form.
